While the physical exam is offered free by the Foundation for new hires, I understand that failure to meet scheduled appointments or taking required test could result in my being billed for such tests or services.
I understand that my employment is AT WILL, and that either party is free to terminate the employment relationship at any time without cause. Should I terminate employment, I will give notice and follow applicable policies and procedures as necessary. I also understand that my employment may be terminated for any misstatement or omission of fact appearing on this application form.
If employed, I will be required to complete an Employment Verification Form (I-9) and within three days show satisfactory evidence of identity and eligibility for employment.
Applicants are advised that positions are posted in-house one week, during which times McCready Health associates have the opportunity to bid on openings. Applicants remain on file for six months. You must renew an application after this time to be considered for opening.

The data collected as part of this application will be used as part of the pre-employment process, for the conditions of employment listed above and for the employee record upon hire. As mentioned above, applications remain on file for a year. In the event of a breach of unsecured information, if your information has been compromised, it is McCready Health’s duty to notify you.
